Outbound referrals go out on the standard broker-to-broker agreement, and client communication beats the two-day rule TREC sets rather than meeting it.

One thing ends the relationship, and it is dropping the ball on a lead. A slow month does not. A deal that falls apart does not. A lead somebody handed you that you let sit does.
